The Opportunity
Our client is seeking a Registered Nurse - Project Coordinator to join their company in Orange, NSW, with the potential to earn an excellent negotiable salary that is commensurate with your experience.
The primary purpose of this position is to work in a Project Coordination role to support people with chronic disease and other complex healthcare needs, to access mainstream health care providers under a regional Hospital in the Home project and Integrated Team Care program.
Reporting directly to the Practice Manager some of your responsibilities include:
- Oversee & Manage the Hospital in the Home and Integrated Team Care Projects
- Assist the organisation to improve the health of Aboriginals & Torres Strait Islanders
- Provide support to other registered nurses and aboriginal health workers
- Ensure clients with diagnosed chronic conditions receive coordinated care
